Fix nullable rate fields for pass-through fees in settlements

In the settlement cost object, rate.fixed and rate.percentage are now documented as nullable. For pass-through fees, which are charged by an external party rather than at an agreed Mollie rate, both fields are null. API behavior is unchanged.
